If you are a new patient, visiting Florida Hand Center for the first time, then you must print each of the forms below, fill out the information and bring them with you to your initial appointment at our hand clinic. We appreciate you filling them out completely prior to your arrival, in an effort to reduce patient wait times. If you are unable to fill out the forms below, then we kindly ask that you arrive 30 minutes early for your scheduled appointment. We make every effort to ensure that you are seen in a timely fashion, but incomplete and missing paperwork will delay your appointment time.
